If there is more than one Membership ordered, please write each person's NAME and EMAIL in the Instruction/Special Request box at the Checkout stage
 
Completing Multiple requirements:
If you are ordering multiple Memberships with different merchandising items please add 2 separate lines. Also, if you wish to purchase a Membership plus additional merchandising items these need to be ordered as separate lines.
